Prostate Cancer: Next-Step Decision Tool

Based on the EAU Guidelines on Prostate Cancer. Decision support for clinicians; not a substitute for MDT discussion or clinical judgement.

1. Patient fitness and life expectancy

Frail / "too sick" after geriatric assessment (or ECOG 3-4)
Charlson Comorbidity Index calculator (tick every condition the patient has)
Do not count the prostate cancer being treated. Count another separate malignancy only if present. Age is handled separately by the life-expectancy model.
Charlson score: 0
G8 geriatric screening calculator (recommended at age 70+)
G8 score: 0 / 17
Life-expectancy calculator: Gompertz model of male all-cause mortality, adjusted by comorbidity (x1.4 hazard per Charlson point), G8 ≤14 (x1.5) and frailty (x2.5). This is an approximation for decision support, not a substitute for national life tables or a formal geriatric assessment. If you enter your own estimate above, it overrides this calculation.

2. Disease characteristics

Oligometastatic (few lesions, ideally on PSMA PET)
Brachytherapy unsuitable (gland >60 cc, severe LUTS/IPSS >20, prior TURP, poor anatomy)
Unsuitable/unwilling for surgery (anaesthetic risk, preference)
Fit for docetaxel chemotherapy (metastatic setting)
mpMRI and (if applicable) PSMA PET already done
